Realm Crafter is releasing it's pro version on December 2, 2008. This pro version allows you to modify the source code in C++. You should be able to add all sorts of interesting things to this Engine.
Edit: I might have that wrong. Pro is releasing, and it will be a DX9 Engine, and it is in C++ but I'm not sure if you can edit the source code or not.
